Other than Chrissie and the two Charlottes, who only signed last week, this is now unacceptable, as I have been constantly asking for these forms since the beginning of July. I am aware some of you have been on holiday, at work, or unable to get the forms to me, but these forms are important, especially as they contain vital emergency contact details and medical information which the club must keep on record under FA rules.
The Club Registration form can now be downloaded from the website via here.
If your name is not listed above, please download the form, fill out the front in full, read FULLY and then sign the last page and return the full document to me ASAP.
I will inform the committee at the first meeting on 6 September if anyone has failed to submit the form by then.
